NeuroAxis Health Evaluations

At NeuroAxis Health, we take an evidence-based approach to understanding complex neurological conditions along with sport and spine injuries. The practice was founded by Dr. Rachman Chung, DC, DACNB, FACFN, QEEG-DL; and is supported by our care team led by Dr. Jessica Tolentino, DC, QEEG-DL, an expert in neurofeedback. Together, we provide thorough, patient-centered evaluations that guide personalized care.

Functional Neurology Evaluation

Duration: 3 hours

The Functional Neurology Evaluation is the first step in your journey. Over three visits, we measure how your brain processes information across sensory, motor, and cognitive systems. This helps us identify where your symptoms originate and build a treatment plan tailored to your needs.

What We Measure

This evaluation examines seven key areas of brain and body function::

  • Brainwaves (qEEG): measures electrical activity in the brain and can identify patterns linked to sleep, mood,cognition and sensory-motor systems.
  • Neuro-Cognition: measures and tracks attention, memory, psychomotor speed, reaction times, executive function and behavioral symptoms.
  • Oculomotor: evaluates eye movement control and brain pathways linked to vision, balance and focus. 
  • Vestibular: assessment of balance and inner-ear reflexes related to posture, motor control and orientation.
  • Somatosensory: examination of musculoskeletal, orthopedic and sensory-motor systems.
  • Autonomic: evaluates how the autonomic nervous system regulates cardiovascular, respiratory, and stress responses in the brain and body.
  • Physiology: lab testing ordered when needed to evaluate metabolic, immune, hormonal and nutritional factors linked to recovery and optimal wellness.

After the Evaluation

You’ll receive a customized plan that may include neuromodulation, neurofeedback, vestibular and neurological rehabilitation, chiropractic care, biofeedback, and lifestyle guidance. Our goal is to support neuroplasticity—your brain’s ability to heal and adapt—so you can achieve long-term improvements in health and function.

Specialized Evaluations

Acute Vertigo Evaluation

For sudden onset dizziness or spinning sensations. This focused evaluation identifies the most common causes of acute vertigo and often allows for same-day treatment.

Includes: health history, focused neurological exam, videonystagmography (VNG), positional testing for BPPV, treatment maneuvers, and home therapy guidance.

Common Conditions: BPPV (Benign Paroxysmal Positional Vertigo), vestibular neuritis, acute vestibular migraine.

Chronic Dizziness Evaluation

For patients with long-term dizziness, balance problems and motion sensitivity. This evaluation examines the visual, vestibular, and proprioceptive systems to uncover persistent causes of dizziness or unsteadiness. In complex cases, we may recommend a full Functional Neurology Evaluation.

Includes: health history, physical and neurological exam, visual-oculomotor and vestibular testing, computerized posturography, and home therapy guidance.

Common Conditions: vestibular neuritis, vestibular migraine, post-concussive dizziness, Persistent Postural Perceptual Dizziness (PPPD), Mal de Debarquement Syndrome, Meniere’s disease, and cerebellar ataxia.

Neuro Plus Evaluation

Designed for those experiencing sleep, mood, cognitive, or -related challenges. This deeper evaluation is ideal for patients who may have already had vestibular or vision testing and want more insight into brain function and personalized neurotherapy program.

Includes: health history, neurological exam, qEEG brain mapping with full analysis, and a review of findings with treatment recommendations.

Common Conditions: anxiety, depression, OCD, PTSD, cognitive impairment, traumatic brain injury, vestibular migraine, PPPD, insomnia.

Migraine Evaluation

A comprehensive look at chronic or recurring migraines, assessing neurological, physiological, and lifestyle factors that contribute to headaches. This evaluation helps uncover root causes and provides a clear treatment path.

Includes: health history, neurological exam, qEEG brain mapping, review of findings, and customized treatment plan.

Common Conditions: chronic or episodic migraines, vestibular migraines, stress- or anxiety-related migraines, migraines with cognitive or visual symptoms.

Introductory Neurology Evaluation

For patients with new or unexplained neurological symptoms who are seeking answers and next steps. This general assessment helps determine which diagnostic tests or specialists may be needed.

Includes: neurological exam, referrals for imaging or diagnostic testing (as appropriate), and coordination with other physicians if needed.

Common Conditions: headaches, migraines, concussion, numbness or tingling, peripheral neuropathy, weakness, or complex/unexplained neurological symptoms.

Sport & Spine Evaluation

For musculoskeletal injuries, pain, or performance optimization. This evaluation is tailored to athletes and active individuals, as well as patients managing chronic spine conditions.

Includes: musculoskeletal and orthopedic exam, manual therapy, chiropractic adjustments, and performance recommendations.

Common Conditions: neck and back pain, disc herniations, spinal stenosis, radiculopathy, and joint injuries (shoulder, knee, hip, wrist, ankle).

Before I went to Dr Chung, I was having imbalance issues, unable to move my eyes in certain ways without causing immediate and severe neurological issues, and plagued with insomnia and night terrors. Dr Chung's treatment was literally the ONLY thing that helped improve my symptoms.

Dr. Chung is the best in the city, along with his entire staff who are all so friendly, caring, and knowledgeable! I had a horrible nerve issue that was ongoing for a year post-ankle fracture surgery, and saw 5+ doctors that couldn’t diagnose or figure out why I was having the issue. At one point it was difficult to even walk or day to day activities without feeling chronic pain. Then I saw Dr. Chung, who helped me beyond addressing the root cause - and provided a holistic and long-term treatment. My treatment included advanced red-light therapy, physical therapy, chiropractor sessions, and recommendations for lifestyle changes. In just a few months, Dr. Chung has made such a difference in my life. When I came to Dr. Chung about my migraines, I was at the end of my rope. I’d tried just about every preventative pharmaceutical on the market and was still having migraines multiple times a week, usually requiring triptans for relief. Within weeks of working with Dr. Chung, I was experiencing significantly fewer migraines and today I am happy to report that I’m two months headache-free. I feel better than I ever thought possible and am working with my primary care physician to taper off preventive pharmaceuticals. Dr. Chung is the reason I’m a functional human again. I had 2 serious head traumas that left me severely impaired, and he helped me heal and resume cognitive and neural functions. I was a wreck! Working with Dr. Chung has been revolutionary for my traumatic brain injury. For the last few years, I have been experiencing post-concussive symptoms related to vision, balance, and coordination. I came to Dr. Chung feeling defeated and hopeless and through his in-depth assessments, he put many of the pieces together and provided personalized treatment that targets my weaknesses. Dr. Chung is everything you’d want in a practitioner; he’s kind, knowledgeable, caring and exceptional in his skills. I originally came for migraine help (and haven’t had a single migraine since my first appointment) and am excited to continue the work on this through his very holistic and comprehensive approach (adjustments, Biofeedback and more). Last week I had a serious inflammation in my knee which completely debilitated me. Just left his office with range of motion and significant less pain after he carefully and methodically worked to uncover the source of the injury and how to relieve the pain and adjust the muscles and joints. He also sent me home equipped with gentle exercises to continue the healing. I’ve seen many practitioners in my life and so far Dr. Chung stands out in all the ways that matter when choosing a doctor.
