What’s the Difference Between Dizziness, Vertigo, and Disequilibrium?

Many people use the terms interchangeably, but they describe different experiences:

  • Dizziness: Lightheadedness, floating, or unsteadiness
  • Vertigo: The false sensation that you or your environment are spinning
  • Disequilibrium: A sense of feeling unsteady on your feet

Other symptoms may include nausea, ringing in the ears, brain fog, trouble focusing, or sensitivity to busy environments such as grocery stores.

What Causes Dizziness and Vertigo Symptoms?

Dizziness is not a condition on its own—it’s a symptom that can come from different underlying problems. The causes are categorized in 2 groups:

  • Peripheral causes – issues in the inner ear or vestibulocochlear nerve
  • Central causes –brain and spinal cord regions involved with visual, vestibular and movement.

What Are the Most Common Causes of Dizziness and Vertigo?

Peripherally-Based Conditions

  • Benign Paroxysmal Positional Vertigo (BPPV): Brief spinning episodes triggered by head movements, caused by tiny crystals in the inner ear.
  • Vestibular Neuritis & Labyrinthitis: Sudden, severe dizziness and/or hearing loss from inflammation in the inner ear or vestibular nerve, following a viral infection.
  • Ménière’s Disease: Recurring dizziness with ear ringing, hearing changes, or fullness from inflammation or fluid buildup in the inner ear.
  • Acoustic Neuroma: A rare benign growth on the vestibulocochlear nerve that can cause dizziness and hearing loss.

Centrally-Based Conditions

  • Vestibular Migraine: Repeated dizzy or vertigo attacks, often without headache.
  • Persistent Postural-Perceptual Dizziness (PPPD): Chronic dizziness, motion sensitivity, swaying or rocking sensation following a triggering event or psychological stress.
  • Mal de Débarquement Syndrome (MdDS): Lingering motion sensation after travel by ship, plane, or car.
  • Neurological Disorders: Stroke, multiple sclerosis, neurodegenerative conditions, and other conditions that affect visual and balance pathways.
  • Head Injuries or Brain Tumors: Trauma or growths that disrupt vision and balance centers.

⚠️ When to Seek Immediate Care: If dizziness comes on suddenly with double vision, speech difficulty, weakness, numbness, or loss of consciousness, call 911 right away.

Dr. Chung is the best in the city, along with his entire staff who are all so friendly, caring, and knowledgeable! I had a horrible nerve issue that was ongoing for a year post-ankle fracture surgery, and saw 5+ doctors that couldn’t diagnose or figure out why I was having the issue. At one point it was difficult to even walk or day to day activities without feeling chronic pain. Then I saw Dr. Chung, who helped me beyond addressing the root cause - and provided a holistic and long-term treatment. In just a few months, Dr. Chung has made such a difference in my life. When I came to Dr. Chung about my migraines, I was at the end of my rope. I’d tried just about every preventative pharmaceutical on the market and was still having migraines multiple times a week, usually requiring triptans for relief. Within weeks of working with Dr. Chung, I was experiencing significantly fewer migraines and today I am happy to report that I’m two months headache-free. I feel better than I ever thought possible and am working with my primary care physician to taper off preventive pharmaceuticals. Working with Dr. Chung has been revolutionary for my traumatic brain injury. For the last few years, I have been experiencing post-concussive symptoms related to vision, balance, and coordination. Dr. Chung is everything you’d want in a practitioner; he’s kind, knowledgeable, caring and exceptional in his skills. I originally came for migraine help (and haven’t had a single migraine since my first appointment) and am excited to continue the work on this through his very holistic and comprehensive approach (adjustments, Biofeedback and more). Last week I had a serious inflammation in my knee which completely debilitated me. Just left his office with range of motion and significant less pain after he carefully and methodically worked to uncover the source of the injury and how to relieve the pain and adjust the muscles and joints.
